Boarding Authorization & Agreement Terms *

I, the undersigned owner of the above mentioned dog, hereby authorize and consent to the boarding of my pet at the designated facility. I confirm that my pet has been vaccinated as required and that all information provided is true and correct. I acknowledge that there are inherent risks in boarding my pet and release the facility and its employees from any liability for injury, loss, or damage to my pet while boarding. I understand that any veterinary expenses incurred during my pet's stay will be my responsibility and I authorize the facility to obtain veterinary care for my pet if necessary. I authorize the facility to contact the emergency contact person listed above in the event of an emergency or if I cannot be reached. I acknowledge that I will be responsible for all boarding fees and agree to pay all fees upon my pet's departure.
